For a female Sim, becoming pregnant is fairly straightforward: choose the Try for. Other problem is that sims 3 female body mesh is already low poly so trying to increase belly's size can generate a blocky result (the original pregnant mesh by EA already has hard edges), plus clipping and bones shenanigans. You can create clothes with a modified pregnant morph to show a bigger belly, as everybody said making it work with a script would be too laborious but Cmar's Pregnancy Controller helps a little.
